Making Appointments:
To schedule an appointment with a Dearborn Surgical Associates physician, please fill out and submit the information below:

New Patients:
If you are a new patient and have been referred by another physician, please have your doctor's referral available when you call to schedule an appointment or test. You will receive a short medical questionnaire to complete prior to your first visit. Please review the "First Visit" information posted on this web site.
Important: HMO Members will need to have a referral form generated by their primary care physician.

Existing Patients:
If you are an established patient, please try to call for an appointment at least two weeks before you need to be seen.
Note: While we make every effort to meet all of our scheduled appointments, it is sometimes difficult to predict exactly how much time a patient's care will require, and occasionally important interruptions occur. When delays arise, you will be notified as soon as possible.

Canceling Appointments:
We appreciate at least 24-hour notice if you need to cancel your appointment. This helps to ensure the best possible care for all patients.

Emergencies:
In case of an emergency, call 911 to dispatch an ambulance. It is suggested that you advise the ambulance driver to take you to Oakwood Hospital Emergency Room. Once you are admitted to the emergency room, staff can notify the Dearborn Surgical Associates physician on call.

After Hours:
During non-office hours please call 313-271-8560 to contact a Dearborn Surgical Associates physician. Your phone calls can be received 24 hours a day by our answering service. Please explain your requests or concerns to the operator so they can relay the proper information to the physician on call. Once the information is received, our physician on call will return your call as soon as possible.
